Job Description:
The LPN/LVN provides skilled bedside nursing care to terminally
ill patients, following the individualized care plan and under RN
supervision.
Evaluates patient and family needs, assists in care planning, and
implements treatments and medications.
Monitors patient reactions, reports changes to the team, and
documents clinical notes accurately.
Maintains a safe environment, supports patient comfort through
various nursing protocols, and participates in patient and family
teaching.
Contributes to interdisciplinary team efforts and fosters
therapeutic relationships to promote patient independence.
Participates in on-call rotations, flexible scheduling, and
professional development activities.
Requires current practical nursing license, one year of hospital
medical-surgical experience, reliable transportation, and physical
ability to perform duties.
