Registered Nurse (RN) | Case Manager Location: Seattle, WA Agency: Mindlance Health Pay: $2,503 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 26 Weeks Start Date: 2/9/2026 About the Position ID: 74362184 Shift: Day 5x8-Hour (08:00 - 16:30) Description: Candidate must be open to going perm after the initial contract periodPENDING WA LICENSE OKThe RNCM establishes a well-rounded care practice that includes: a patient assessment, care planning, implementation, coordination and monitoring, and ongoing routine evaluation that ensures medically and financially appropriate care. Responsibilities includes: facilitating interdisciplinary team care coordination, intensive medical management of participants recently discharged from acute care centers and skilled nursing facilities. Additionally, this position will work closely with the IDT Social Worker and Physician to collect necessary medical and social information to ensure cohesive decisions are made which ensure the safety and well-being of participants. This position is primarily on site, but there are times when RNCM will need to drive to a participants residence for an assessment if needed. Mileage will be reimbursed for this time. RNCM will start from the center and leave to the community to do those visits. Providence PACE is a Program of All-Inclusive Care for the Elderly that strives to keep older adults as healthy as possible living in the community through clinics, home visits and more. If required for the assignment, Client agrees to supply Providers with communication devices (i.e. cell phone, pager, etc.) needed to perform the duties as assigned at no cost to Provider or Agency. If Client does not provide a mobile phone required for the assignment, Client agrees to pay Agency monthly, for all work-related cell phone use by each Provider, up to a maximum amount of $50.00 as invoiced by Agency. Providers will be responsible for submitting monthly cell phone bill directly to Agency. For candidates on 48-hour schedules, OT after 48 hours in a week Modules: Modules are non-billable, average 16 hours spent.
