or ESKD Focused Primary Care Join Us in Building a New Model of
Kidney Care – Washington Locations We’re launching an innovative
pilot program to transform how Nurse Practitioners (NPs) deliver
care to patients with Chronic Kidney Disease (CKD) and End-Stage
Kidney Disease (ESKD). We’re seeking dynamic, forward-thinking NPs
to pioneer this new model of primary care—providing proactive,
integrated services alongside nephrology teams. Open Locations:
Seatle Tacoma Bellevue Bellingham We have two NP roles: one focused
on ESKD care and one focused on CKD care. What You’ll Do As an NP
with IKC, you’ll serve as a key provider for high-risk patients in
either CKD or ESKD stages. Working closely with nephrologists,
dialysis teams, and interdisciplinary staff, you will: Provide
primary care within dialysis units or nephrologist offices Manage
chronic illness and co-morbid conditions such as heart failure
(HF), hypertension (HTN), and diabetes (DM) Reduce hospitalizations
and total cost of care through preventive, patient-centered
approaches Help establish and grow new outpatient CKD clinics near
dialysis units Champion care redesign in an innovative,
patient-first environment Ideal Candidate Profile We’re looking for
NPs who are: Excited to be part of a trailblazing pilot program
Independent, resourceful, and ready to help stand up a new model of
care Experienced in nephrology, primary care, or dialysis settings
Interested in growth and future leadership opportunities For
ESKD-Focused Roles: Experience in dialysis and fluid/access
management Strong understanding of managing patients in the
dialysis setting Focus on preventing hospitalizations and
coordinating wraparound care For CKD-Focused Roles: Proficient in
managing comorbidities: HF, HTN, DM Comfortable with palliative
care discussions and advance care planning Familiarity with

Requirements Minimum 2 years’ NP experience Unrestricted NP license
in Washington DEA license (or eligible within 90 days) BLS/CPR
certification Intermediate proficiency in Microsoft Office
Experience with Medicare Advantage, population health, or
