for professional licensure Job Summary The Community Health Worker
– School Based facilitates access to ICHS school-based health
centers and primary care clinics to connect youth and their
families to quality affordable care. This role will support
referrals and linkages to external and internal services for
school-based patients. This role will also provide operational
support for the school-based health center. Additionally, this
person will identify and maintain relationships with school
partners who serve populations with identified health disparities.
Activities may include promoting access to preventive care through
independent outreach and education with individuals or groups in
the community. They will also provide support with insurance
enrollment. Education - Associate’s degree. Experience in a health
care setting may be substituted year for year for the education
requirement. Experience - One (1) year experience in a health care
setting, community health education and/or outreach position. Other
Requirement(s) - Fluency in English is required: additional fluency
in another language is valued. Proficient computer skills.
