Summary:Responsible for providing direct age specific and
culturally appropriate care through the use of the nursing process,
based on evidenced base care, and utilizing the scope of Practice
as defined by Washington State law, the ANA Scope and Standards of
Nursing Practice within the urgent care environment. The Nurse will
work collaboratively with the Urgent Care Manager and the Medical
Director in developing and maintaining urgent care protocols,
policies and procedures. Additionally, the Urgent Care Nurse will
provide direction to the non-physician clinical team and educate
patients and families on health education, disease prevention and
chronic disease management.Your contribution to the team includes:
Identify and assess high-risk patients using established criteria,
coordinate multi-disciplinary teams and resources to support
patient care needs, provide triage for patients across the life
span. Executes nursing interventions by collaborating with the
inter-professional team incorporating appropriate standards of care
and practice and patient outcomes. Assesses and systemically
collects any data related to patient health status and works in
collaboration with the care team to develop the plan of care.
Evaluates patient's response to plan of care and works in
collaboration with the inter-professional team to make any needed
adjustments. Assumes responsibility and accountability for
effectively managing nursing care of individual patients. Documents
nursing assessment and interventions to reflect comprehensive and
integrated approach to patient care. Actively participates in the
team care of patients with complex chronic diseases, coordinates
care of complex patients, provides preventative health. Assist in
smooth transitions of care between HealthPoint in-patient and ED
settings. Maintain ongoing communication with patient/family and
care team. Acts as an advocate for patient care needs. Ability to
provide telephone triage and prioritize interventions based on
phone triage assessment. Collaborations with PCP, MA, clinic
managers and those who are responsible health care team members.
Inform patients of abnormal lab test and x-ray results. Provide
support and education of Medical Assistants that includes
mentoring, training and oversight in basic skills and procedures,
testing knowledge base and giving in-services.Must have's you'll
need to be successful: A Graduate of an accredited school of
nursing program required with a minimum of two years of recent
clinical experience in an ambulatory setting. Emergency Room, ICU
or UC RN experience will be preferred but not required. Enjoy
working in a fast-paced patient focused environment. Excellent
interpersonal/people skills. Strong organizational/multi-tasking
skills. Familiarity with work with medical interpreters is highly
desired.Proof of immunizations and/or immunity to: COVID-19, MMR
(measles, mumps, and rubella), Varicella, Annual Influenza, and TB
are required for all employees. Additionally, if you work in a
HealthPoint clinic, Tdap (within last 10 years) is required.
Hepatitis B. is required for clinical employees with potential
exposure to blood/blood products. All immunizations are a condition
of employment. Upon hire, employees must provide proof of their
immunizations and/or immunity prior to starting or no later than
their fifth (5) business day of employment.Where to gather your
