ASAP About the Position Pay Rate: $44.00 - $48.00/Hourly. Summary:
The Licensed Master Social Worker (LMSW) provides psychosocial
assessment, care coordination, and supportive counseling services
to individuals and families. The LMSW collaborates with
interdisciplinary teams to address social, emotional, and
environmental factors impacting patient well-being and supports
safe, effective care transitions across settings. Responsibilities:
Conduct psychosocial assessments to identify patient needs,
strengths, and barriers to care. Provide short-term counseling,
crisis intervention, and emotional support to patients and
families. Develop and implement care plans in collaboration with
the interdisciplinary team. Assist with discharge planning and
coordination of community resources and services. Advocate for
patients regarding access to healthcare, housing, financial
assistance, and social services. Requirements Requirements: 1 -
Year experience Required, Epic experience Required, BLS
Certification, LMSW (Licensed Masters of Social Worker). Benefits
